 Here are Kevin’s answers:

1.Man operating the crane while its being worked on.

2. Man operating crane with one hand on controller with headphones in and with a body part under a suspended load while not paying attention. 3. Improperly secured I beam. ( should be 2 straps with choker instead of 1 in basket formation)

4. The crane should be locked out while maintenance is being performed.

5. Contractor working on crane not focusing on the task at hand. Without proper fall protection and loose caution tape dangling from aerial lift platform.

6. Hazardous work area not properly cautioned taped off.

7. Man in red sweater causing a tripping hazard.

8. Man sitting in non designated break zone.

9. Man wearing shorts in the workplace.

10. Man not lifting with both hands or looking in his direction of travel. Possibly watching something inappropriate on his phone.

11. Man on top rung of ladder not holding on with both hands or even paying attention at all.

12. Woman under ladder on her phone wearing opened toed footwear.

13. Sawdust on floor causing a slipping hazards.

14. Nobody wearing steel toes, hard hats, hi viz vests or safety glasses.
